Purpose: Myofibroma is a rare benign spindle cell neoplasm, and the aim of the present study was to carry out a literature review and present a clinical case of a patient with a myofibroma in the submandibular region and its management. Conclusions: Diagnosis of myofibroma can be reached by a histopathologic and immunohistochemical analysis and surgical excision is the treatment of choice.

Our response will specifically address his three major points as follows: 1. The traditional classification based on a distinction between "spontaneous" and "medically indicated" preterm birth proposed by Dr. Lopez has long been debated in obstetrics due to its inherent ambiguity and it is no longer in use. To properly address this issue, the Global Alliance to Prevent Prematurity and Stillbirth task force established a new classification system for the preterm birth syndrome intended for clinical and research use [1].

There are controversial evidence in the literature on the role of comorbid anxiety disorders (ANX) in the improvement of attention-deficit/hyperactivity disorder (ADHD) symptoms with methylphenidate (MPH) treatment. Our main objective was to assess differences in the response to MPH treatment in children and adolescents with ADHD with and without comorbid ANX. We extensively evaluated response to MPH in a naturalistic study of 280 children and adolescent with ADHD according to DSM-IV criteria. Psychiatric diagnoses (ADHD, ANX, and other comorbidities) were assessed by semi-structured interviews (K-SADS-E). Response to MPH was assessed by means of total score in the Swanson, Nolan, and Pelham Scale-version IV (SNAP-IV) after 1 month of treatment. There was no significant between-group difference in the response to treatment with MPH after 1 month either when SNAP-IV scores were assessed dimensionally or categorically (moderate response) (P > 0.05). Our findings suggest that comorbid ANX do not interfere in the response to MPH on core ADHD symptoms.

Construction and demolition waste (CDW) is heterogeneous and construction aspects affect their performance. Aiming at better understanding CDW and providing data for rational pavement design, this study examines factors influencing CDW mechanical behavior for base and sub-base layers. CDW materials from three sources were evaluated using shear strength and resilient modulus tests. Results indicate that CDW composition affects the mechanical behavior, especially ceramic particles, which easily break and increases deformability. Proper construction (wetting process, compaction method, relative compaction), and composition selection (chemical stabilization and increased concrete particles content) enhance CDW performance. These factors collectively affect CDW behavior and must be considered in pavement design.

Objective: With the assumption that circadian rhythms influence human performance, the work of live line electricians was reorganized and evaluated.The hypothesis was that in highly physical and attention-demanding work, the organization of tasks, according to the ideal period of day and day of week, should diminish stress and consequent work risks. Background:There are only a few studies reporting the work of electricians and even fewer approaching work organization. Moreover, these investigations often do not consider human physiological limitations and capabilities as well as task demands. Method: A new work system was proposed with consideration of (a) the circadian cycles and homeostatic processes; (b) the effect of heat, which is a zeitgeber (synchronizer) for the biological clocks; and (c) the degree of physical and mental demands of the different performed tasks, which was assessed on the basis of opinions of the electricians and physiological markers of stress that are controlled by circadian rhythms. The traditional and new systems were compared on the basis of two cognitive indices (the arrangement of matchsticks and the perception of a minute) and three physiological markers of mental-to-physical loads (heart frequency and the level of adrenaline and noradrenaline). Results: Both physical and mental loads were reduced in the new system. Conclusion: Work organization should include consideration of human circadian rhythms, mainly when stressful and high-risk tasks are involved. Application: The findings can be applied in any work design, but they are especially suited for highly demanding work carried out outdoors.

Processing-in-Memory (PIM) or Near-Data Accelerator (NDA) has been recently revisited to mitigate the issues of memory and power wall, mainly supported by the maturity of 3D-staking manufacturing technology, and the increasing demand for bandwidth and parallel data access in emerging processing-hungry applications. However, as these designs are naturally decoupled from main processors, at least three open issues must be tackled to allow the adoption of PIM: how to offload instructions from the host to NDAs, since many can be placed along memory; how to keep cache coherence between host and NDAs, and how to deal with the internal communication between different NDA units considering that NDAs can communicate to each other to better exploit their adoptions. In this work, we present an efficient design to solve these challenges. Based on the hybrid Host-Accelerator code, to provide fine-grain control, our design allows transparent offloading of NDA instructions directly from a host processor. Moreover, our design proposes a data coherence protocol, which includes an inclusion-policy agnostic cache coherence mechanism to share data between the host processor and the NDA units, transparently, and a protocol to allow communication between different NDA units. The proposed mechanism allows full exploitation of the experimented state-of-the-art design, achieving a speedup of up to 14.6× compared to a AVX architecture on PolyBench Suite, using, on average, 82% of the total time for processing and only 18% for the cache coherence and communication protocols.

Introduction This study aimed to characterize the dimensions of a selection of 3-rooted maxillary premolars through high-resolution computed tomographic analysis considering measures of clinical interest for root canal treatment, such as root wall thickness, canal diameters, and distances between 2 canals in fused roots and between the root apex and foramen. Methods Fifteen 3-rooted human maxillary premolars extracted for therapeutic reasons were individually scanned using a high-resolution desktop high-resolution computed tomographic system. Starting from the apical foramen, the selected cross-sectional images corresponding to each millimeter of the roots were evaluated. Measures of clinical interest were determined using Image J software (version 1.41; National Institutes of Health, Bethesda, MD). Results Root wall thickness as thin as 0.4 and 0.6 mm was detected in the apical portion of buccal and palatal roots, respectively. In the cervical portion, buccal roots had narrower root walls (0.817-1.670 mm) compared with palatal roots (1.361-2.720 mm). In all thirds, the palatal canal was wider compared with the mesiobuccal and distobuccal canals. In the buccopalatal direction, all roots had thicker root walls toward the furcation, whereas in the mesiodistal direction the mesiobuccal and distobuccal roots had the thinnest walls along their distal and mesial aspects, respectively. Both buccal canals revealed dentin apposition 2 mm from the canal orifice, resulting in cervical constriction. Generally, the distance between the root apex and the foramen was greater in distobuccal roots in comparison with the others with a tendency for foramina to be eccentric. Conclusions Three-rooted premolars are a clinical challenge not just because of their low frequency and difficulties regarding diagnosis and root canals access but also because of their fragile roots. These phenomena are critical in terms of the amount of dentin removed during the preparation of root canals and during post space preparation.
